as a patient in the practice. Click upper right tab on this page and follow prompts. One cannot be scheduled until this step has occurred. Be sure to include all information requested, including the Biographical Statement as well.
2. Then go to POLICIES tab, download and complete the "Initial Paperwork"
and bring to your appointment [see Policies
]. Please try to arrive 5 minutes early to complete the OQ-45 Questionnaire
on my clipboard in the reception area.
3. After the initial session, go to Therapyapointment.com to schedule, reschedule, or cancel future appointments.
NOTE: If you are filing insurance, you are responsible for determining if your policy deductible is met and your required amount for payment. Otherwise, you will pay practice fees and then be reimbursed.
