Empirically-supported outcome measures (OQ-45.2, Session Rating Scale/Outcome Rating Scale) are consistently employed to measure progress and goal acheivement. Note: All receiving clinical services will be assessed a Semi-Annual Practice Fee of $150.00.
If filing with insurance, patients are responsible for determining if deductible's are met and the required co-pay.
