Contact Us (Appointments)
To schedule an appointment with Dr. Frontman, please complete the information below. Be sure to include any additional information in the message box below. If you prefer, you may contact us via telephone to schedule an appointment at (678) 624-0930, ext. 16.
Office Hours: Dr. Frontman's practice hours are on Mondays (9 a.m. -7 p.m.), Tuesdays (11 a.m. -8 p.m.) and Fridays (1 p.m. -5 p.m.).
Cancellations: All appointment cancellations must be made at least 24 hours in advance, including for the initial appointment. Failure to do so results in a $60 fee ($50.00 for group) that can only be billed to the patient.
